What to do next:

1. Read the two letters and complete form 1 & 2
2. Return form 1 to reception and book a new patient appointment with the Practice nurse/HCA
3. Bring form 2 and a urine specimen to the new patient appointment. If you are on any repeat medication, please bring your prescription or medication with you
