Summary Care Record - in brief.
If you are a patient of Dr J R Buckle & Partners Practice, you will have received a letter about the Summary Care Record. The letter contained details regarding the upload of certain information from your clinical record to your online Summary Care Record. Below is a brief, simplified guide to hopefully help those that are still unsure what it is all about.
What will be uploaded?
Your SCR will consist of your Medication (current and discontinued), Allergies and Drug Sensitivities. Your full clinical record will be retained only with your registered practice.
Who can access it?
Only those connected to the NHS computer network.
How will be used?
If you are conscious, you may be asked for permission to access your SCR in any health centre or Hospital. You can refuse them access.
If you are unconscious, paramedics or A&E may access your SCR to get a better picture of your conditions.
What is it for?
To allow a greater insight for other doctors or paramedics about your Medication, Allergies and Sensitivities that do not have access to your full clinical records.
Do I have to have one?
In short, no. You can use a form or apply in writing to your registered practice to ‘opt out’ of the upload of your Medication, Allergies and Sensitivities. You will then have a blank sheet as your SCR. You can ‘opt in’ in the future if you change your mind.
